  Have you given USBclean Full
  Disk Access in system settings? Also make sure to open USBcleab settings and choose which file types you would like removed

  ok, It was moved to a new location:
  /System/Library/CoreServices/Applications/Keychain Access.app

  you can easily open it from Spotlight though (spacebar + command) keys

  thanks for the crash log, unfortunately it doesn't provide me any clues... Do you see any pattern to the crashing?

  If you look for any Cookie crash logs in the folder:

  Try updating to macOS 15.1.1, I’ve had feedback from someone that the issue was fixed after updating macOS

  @FrozenDude Ive just released v4.1 on my servers with a new Exceptions sub-tab on the File Types tab in Settings.

  you can add ._. to the token field to preserve these file types during a clean.
